REQUIRING THE OFFICIALS AND EMPLOYEES IN THE OFFICE OF THE PRESIDENT TO ACCOMPLISH A PERSONNEL ASSESSMENT FORM.

In connection with the on-going audit of personnel of the Office of the President and the establishment of Promotion Boards therein as well as the proposed creation of a Personnel Data Bank in the Office, there is need to gather and compile all and airy information about all officials and employees.

It is therefore hereby directed that all officials and employees in the Office of the President accomplish the Personnel Assessment Form, copy attached, which shall be submitted to the Personnel Division, Administrative Office, not later than June 15, 1979. The accomplished Personnel Assessment Forms shall be compiled by the Presidential Personnel Officer who shall devise a master personnel record filing system in which shall be entered all the information contained in the Personnel Assessment Forms. He shall be responsible for keeping the said record file up-to-date by calling from time to time for submission of any changes in, and/or additional information to, those contained in the previously accomplished personnel assessment forms.1aшphi1

All heads of offices and primary units are hereby enjoined to cooperate or otherwise assist the Presidential Personnel Officer towards the development and maintenance of a master personnel record system for the Office of the President.
